A clean moving invoice has eleven parts: your company name and DOT or state mover number, the customer with both pickup and delivery addresses, an invoice number and date, the move type, crew size and hours, travel or truck fee, materials, any stairs or long-carry charges, the subtotal, the deposit paid, and the balance due with terms. A stairs or long-carry fee nobody mentioned at pickup turns into a standoff in the driveway. Note possible surcharges on the quote, then itemize the real ones on the invoice.
If the customer has to find a checkbook on moving day, you have invited a "I'll mail it" that takes weeks. Card or ACH on delivery collects the balance before the crew leaves.
"Labor — $2,250" gets questioned. "3 movers × $150 × 5 hrs" gets paid. Showing the rate, crew, and time removes the reason to push back.
A deposit reserves the date and weeds out no-shows. Show it as a credit on the invoice so the balance is right and nobody double-counts what they paid.

Local moves are usually hourly — state the crew size, rate, and hours plus a travel fee. Long-distance is usually a flat or weight-based quote. Write it the same way you quoted it so delivery holds no surprises.
Interstate movers carry a USDOT number and many states require a license number on the paperwork. Even where it is optional it reads as legitimate. Put it near your company name at the top.
